This 4 1/4″ square tile appears to have been removed from an installation. An online search indicates it may have been produced by Monarch, but this sample is unmarked.

While this tile appears to have actually been attached in an installation, other collections that include this tile show markings on the back consistent with a drink coaster or trivet.
